About twenty years ago when an aquaintance of mine (no friend for he was unstable and volatile aeven then) decided to get a sex change he had to convince a panel of doctors and psychiatrists that he seriously believed he was a woman.
not that he was, of course.
but that he believed it.
and he fooled them.
After the op - he would proudly clutch his crotch to demonstate that nothing was left own there - , he would get violent to anyone who called him by his real name. He boasted in my hearing of kicking the stuffing out of any who disrespected his new and fictional gender.
a vietnam vet, a little older than me, he knew how to be violent to vicous effect
and the police there (this was an open air forum, a speakers corner in my home town square ) just smiled and let him get away with it
he had admittted to me and any others in earshot his issues with abandonment and despair so it is an easy inference that the despair and utter self rejection was behind this insane decision.
one thing i have learned since then is that it really appears that transgendered types are desperate attention seekers, which figures for deep alienation from self wil cause this.
And this man was an exibitionist as well
my point is that these days no one seems to bother to establish, even in their own warped terms that the transgender candidate has any bona fide belief involved.
and if we assume for the sake of argument that transgenderism is real ( again i say it is not) then this failure to sort "real" transgenders from the delusional is a gross dereliction of medical ethics
but that of course in this post covid age is to be expected
